This is Ultimate Ninja Storm 4: Road to Boruto, the expanded release that adds the Boruto storyline on top of the complete Storm 4 game.

The additional content picks up after the manga's conclusion, following Naruto's son through the Chunin exams, and adds Boruto, Sarada, Mitsuki and an adult Naruto and Sasuke to the roster. Everything from the base game is here too, including the full Fourth Great Ninja War arc.
